Registering To Vote - Voter Registation Application.pdf
To register to vote, please come to the Caledonia Village Hall, 6922 Nicholson Road, between the hours of 8:00 a.m. and 5:00 p.m., Monday through Friday. On election day only, you may register to vote at your polling place.
You must certify that you are a qualified elector, a U.S. citizen, at least 18 years old or will be at least 18 years old at the time of the next election, having resided at the above residential address for at least 10 days immediately preceding this election, not currently serving a sentence including probation or parole for a felony conviction, and not otherwise disqualified from voting. You must certify that all statements on the application form are true and correct. If you provide false information you may be subject to a fine or imprisonment under State and Federal laws.
Forms of identification which constitute proof of residence when they contain the information specified above include the following:
1. A current and valid WI Driver's License/ID card.
2. Any other official identification card or license issued by a Wisconsin governmental body or unit
3. An employee ID card with a photograph, but not a business card.
4. A real property tax bill or receipt for the current year or the year preceding the date of the
election.
5. A residential lease (Does not count as proof of residence if elector submits form by mail.)
6. A university, college or technical school picture identification or fee card.
7. A utility bill for the period commencing not earlier than 90 days before the day registration is
made.
8. A bank statement.
9. A paycheck.

You must be a registered voter to receive an absentee ballot. If you are a registered voter and wish to receive an absentee ballot, please come to the Caledonia Village Hall, 6922 Nicholson Road, between the hours of 8:00 a.m. and 5:00 p.m., Monday through Friday to complete the application form. Applications may also be submitted by mail to the Village Clerk at the Village Hall. In order to receive the absentee ballot by mail, the application must be received by the Village Clerk no later than 5:00 p.m. on the Thursday before the election.
