Immunization Information

Parents or Legal guardians MUST bring their child’s immunization records with them, or the immunizations will NOT be given.

Immunization Fee

Residents- There is a $7.00/per person administration fee for immunizations given to all Caledonia, Mt. Pleasant, North Bay, and Sturtevant residents.

Non-Residents- Administration fee is $10.00/child and adult immunizations.

• Adult immunizations at $7.00 for Td, $45.00 for Hep B
• TB skin tests (by appointment only) $7.00.
• Flu is offered during the Flu and cold season, at a $30.00 charge, and for those that qualify for Medicare Part B, we are able to bill.
• Pneumonia vaccine is offered all year long, at a $45.00 charge, and for those that qualify for Medicare Part B, we are able to bill.

If you are on Badgercare or Medicaid, this administration fee will be waived.

2 Months

          Hib = Haemophilus Influenza b (meningitis)
          IPV = Inactivated Polio Vaccine

          PCV7 = Pneumococcal Conjugate

4 Months

          DTaP = Diphtheria, Tetanus & acellular Pertussis
          Hib = Haemophilus Influenza b (meningitis)

6 Months

          DTaP = Diphtheria, Tetanus & acellular Pertussis

12+ Months

          MMR = Measles, Mumps & Rubella
          Hib = Haemophilus Influenza b (meningitis)

15-18 Months

          DTaP = Diphtheria, Tetanus & acellular Pertussis

4-6 Years

          MMR = Measles, Mumps & Rubella

Every 10 Years thereafter

          Td = Tetanus and Diphtherias
